Why Research Matters for Therapy Dog Teams
You already know what it looks like when your dog walks into a hospital room. The shoulders drop. The tight jaw softens. Someone who hasn't smiled in days suddenly reaches out a hand.
But when a hospital administrator or school principal asks whether therapy dog visits actually work, you need more than a story. You need science. And the good news is the science is there.

What Happens to Blood Pressure During a Visit
One of the most studied effects of animal-assisted interaction is its impact on cardiovascular response. Research published in journals focused on human-animal interaction has consistently documented measurable drops in both systolic and diastolic blood pressure during and shortly after interactions with dogs.
A study conducted in hospital cardiac care units found that patients who received visits from therapy dog teams showed statistically significant reductions in blood pressure compared to patients who received human volunteer visits alone. The effect was not permanent, but it was real and repeatable.
What makes this finding meaningful for handlers is that blood pressure reduction is not just a comfort metric. In clinical settings, even short-term reductions can reduce strain on the heart, lower perceived exertion, and support recovery windows. Your visit is doing biological work inside that patient's body.
Researchers have proposed that the mechanism involves activation of the parasympathetic nervous system. In plain terms, the presence of a friendly dog shifts the body from a stress state into a calmer, more restful state. That shift shows up in the numbers.

How Therapy Dogs Change Pain Perception
Pain is partly neurological and partly psychological. That is not a dismissal of physical suffering. It is actually what makes therapy dog visits so powerful in pain management contexts.
Research conducted with pediatric patients undergoing procedures found that children who interacted with therapy dogs before and during painful interventions reported lower pain scores on standard clinical scales. They also showed reduced physiological stress markers. The children who had dog visits needed less reassurance from staff and demonstrated shorter distress windows after the procedure ended.
Similar findings have been reported in adult oncology settings. Patients receiving chemotherapy who had therapy dog visits during infusion sessions reported lower nausea perception and higher comfort ratings compared to sessions without animal-assisted support.
Pain researchers point to distraction as one mechanism. When a person is focused on a dog, they are not focused on the needle, the drip, or the ache. But the effect goes deeper than distraction. Interaction with dogs is associated with release of endorphins, the body's own pain-reducing compounds. The dog visit is not just taking the patient's mind off pain. It is activating internal chemistry that genuinely reduces how much pain registers.
Anxiety Scores Before and After Animal-Assisted Visits
Standardized anxiety assessment tools have been used in multiple studies to measure how therapy dog visits affect emotional state. The State-Trait Anxiety Inventory, which measures both situational and baseline anxiety levels, has been administered to patients before and after therapy dog visits in surgical waiting areas, psychiatric units and long-term care facilities.
Across these studies, pre-visit anxiety scores were significantly higher than post-visit scores. The reductions were not trivial. In surgical settings, some studies documented anxiety score drops comparable to those achieved with low-dose anti-anxiety medication, without any pharmaceutical intervention.
For handlers working in schools, the findings translate directly. Research in educational settings has shown that children in reading programs that include therapy dogs demonstrate lower reported anxiety about reading aloud, higher engagement, and greater willingness to try again after making mistakes. The dog does not judge. And that matters enormously to a child who is embarrassed about reading in front of others.
Program coordinators should share these findings with school librarians and literacy coordinators. This is the kind of data that gets reading programs funded and expanded.
Do Therapy Dog Visits Affect Hospital Stay Duration
This is the question hospital administrators care about most. Does bringing a therapy dog team into a facility actually move clinical outcomes, or is it just nice to have?
Research in this area is more limited than blood pressure or pain studies, largely because hospital stay duration is influenced by many variables. But some findings are worth knowing. Studies with cardiac surgery patients found that those who received animal-assisted therapy as part of their recovery protocol showed reduced anxiety, improved ambulation willingness and higher satisfaction scores, all of which are factors correlated with shorter stays and lower readmission rates.
A research team working with older adults in rehabilitation settings found that patients who received regular therapy dog visits were more motivated to participate in physical therapy sessions. Engagement in rehabilitation directly affects recovery speed. A patient who looks forward to their therapy dog visit on Thursday has a reason to keep working on Tuesday.
The evidence here is emerging rather than conclusive. Honest program coordinators should present it that way. But the directional findings are consistent, and the cost of a therapy dog visit is essentially zero compared to other clinical interventions. That cost-benefit calculation matters when making the case to administrators.

The Cortisol and Oxytocin Connection
Two hormones appear repeatedly in therapy dog research. Cortisol is the body's primary stress hormone. Oxytocin is associated with bonding, trust and calm. Research has shown that interacting with a dog decreases cortisol levels and increases oxytocin levels in humans.
What is striking is that this effect has been documented in both the person and the dog. Published research on human-dog interaction has found that when a person and dog make eye contact or engage in close physical contact, both experience a measurable oxytocin rise. The bond runs both directions.
For handlers, this is meaningful beyond the science. It is a reminder that your dog is not simply a tool being deployed in a clinical setting. Your dog is a participant in a genuine relationship, however brief, with each person you visit. That relationship has biochemical weight. It is real.
Keeping cortisol low and oxytocin high is directly relevant to immune function, wound healing, sleep quality and pain tolerance. A therapy dog visit is not just emotionally pleasant. It is physiologically supportive in ways that align with recovery goals.

Understanding the research changes how you talk about your work. You are not just a volunteer who brings a friendly dog to cheer people up. You are part of a documented, evidence-supported intervention with measurable effects on blood pressure, pain perception, anxiety and hormonal balance.
That framing matters when you approach a new facility. It matters when you apply for grants. It matters when a skeptical administrator needs convincing. You can speak with confidence because the evidence supports what you already know from experience.
